The old format was email based. Every time someone posted a message, it went into each subscribers inbox, which could become very cumbersome at times. Under the newer format, each subscriber has the option to go to the board to read/browse messages when they wish to do so, or he/she can opt to have some or all messages sent to their inbox. For example, if you're only interested in all things medical, you can subscribe to have emails sent to you only when a message is posted in the medical forum.
